Consolidate comment punctuation and documentation formatting

Group the comment punctuation edits with the guidance distinguishing
complete sentences from headings and fragments. Keep these style changes
separate from the runtime and test behavior changes.

Include the related comment wrapping and Markdown reflow without changing
application behavior.
