Document the heartbeat as a second, independent monitor

The Cronitor section only described the hourly /login liveness check.
Note that the new heartbeat-url (previous commit) catches a failure mode
that check structurally cannot see: a replica that has silently stopped
syncing while the app keeps serving requests normally.
