document finite-medium settings and refresh docs gates

Add descriptions for the medium settings so the generated reference
check passes, and regenerate the GammaLoop reference snapshot. Document
how to select a medium and set the inverse temperature and chemical
potentials, including Fermi-surface localization and the medium-mode
restrictions. Record reviewed verified-scope digests with matching
architecture notes, and cite the rebased callback-dispatch fix commit.
