Preserve checkpoint work across required rank and D stages

Add immutable required-scope amendments, monotone rank/D steering, exact cold-verifier support and cumulative scope monitoring. Validate staged continuation and the combined four-loop control; freeze the restricted R0 D9 campaign without launching it.
