Improve `ERR` trap (#58)

- add `errtrace` so functions inherit the trap
- report on `stderr`, not `stdout` (whoops)
- report the correct file, not the script that may have `source`d it
- use sometimes-clickable `file:line:` syntax
- report which element(s) of a pipeline failed
- report the exit code
- include a stack trace
