Have `hasDoc` check for `DocEmpty`

Previously `MetaAndDoc {doc = DocEmpty}` would pass `hasDoc`, which was exposed
by the previous commit. It now returns `False`. And that required restructuring
`foldExportItemInfo`, because some fields need to be updated even when there is
no documentation.
