Postprocess design doc TOC with a Python script

Move the TOC rewrite logic out of render-design-doc so the shell script stays focused on rendering and asset copying. Replace the inline Perl post-processing with a standalone Python helper that inserts the flattened TOC under a Contents section after Metadata.
